Outreach Coordinator

Description:Provide the outreach coordination and engagement component of the PATH Program to the community and on behalf of New Horizon’s clients in the program.
Duties:1.0 Provide outreach services to homeless persons in the community and to those involved with the criminal justice system.
1.1 Engage individuals literally homeless and at imminent risk for homelessness as defined by current federal PATH application.
1.2 Assess potential PATH clients for eligibility using current federal PATH eligibility criteria.
1.3 Refer any potential PATH client to appropriate community resources, including New Horizons, based on eligibility as defined in 1.2 above.
1.4 Coordinates with direct care staff to secure housing for enrollee’s.
1.5 Assist identified individuals with application for benefits using SOAR practices and guidelines.

2.0 Coordinate with other New Horizons staff to assist those New Horizons clients meeting criteria for the PATH program
2.1 Respond to referrals from Recovery Support Teams & other agency clinical staff.
2.2 Meet regularly with PATH Housing Coordinator
2.3 Attend staff meetings as necessary relevant to individual client needs.

Directly assist with securing all entitlements for identified agency clients who do not have a Case Manager or Recovery Support Specialist.
3.1 Meets with identified individuals to assist with the application for assistance using SOAR practices and guidelines.
3.1 Monitors progress of benefit process and provides assistance as required
3.2 Develops and maintains an effective working relationship with Disability Determination staff.
4.0 C Collect and manage all data required to complete annual PATH Provider Report
4.1 Collect new PATH enrollee data packets from other PATH Project staff each month
4.2 Maintain database of required PATH data set
4.3 Collect relevant required data from CMHC
4.4 Assist in completing Annual PATH Provider Report

5.0 Participate regularly in Shelter Plus Care Team Meetings conducted by the Treasure Coast Homeless Services Council, Inc.
5.1 Attends and participates in scheduled meetings 90% of the time.
5.2 Coordinates with service providers to ensure that referrals meet criteria.
5.3 Assumes responsibility for oversight and ongoing documentation requirements for Shelter Plus Care enrollees who no longer meet medical necessity criteria for recovery support services.
5.4 Reviews ongoing SPC documentation collected from service providers and ensures that documentation is complete and delivered to fiscal agent in the counties assigned where grant is maintained.
5.5 Assists with presenting new referrals to the committee for consideration.

Identify safe & affordable housing opportunities in the community
6.1 Coordinate with other PATH staff and community partners relevant to sharing information regarding landlords, property management companies, and others with available housing
6.2 Identify affordable housing opportunities in the communities and notify service staff of resources available.
6.3 Visit potential boarding homes to identify availability and living arrangement setup.

7.0 Participates in administrative committees and meetings, clinical training, staff development and community relation activities.
7.1 Administrative committee meetings are regularly attended.
7.2 Maintains up-to-date knowledge of clinical area of practice.
7.3 Participates in relevant staff development in-service training for staff.
7.4 Presents in-service training for staff on Shelter Plus Care requirements and provides ongoing technical assistance as needed.
7.5 Provides a variety of community relations/education activities.

8.0 Performs other duties as requested.
8.1 Miscellaneous duties are performed according to instruction.
Qualifications:AA or AS Degree from an accredited university or college.
